Are You Eligible for CHAC Healthcare Services?

Here is a quick checklist to determine if you are eligible for CHAC healthcare services.

If you...

• Are between the ages of 19 and 64
• Are a resident of Chippewa, Luce or Mackinac County
• Do not have health insurance (i.e., Blue Cross or Aflac)
• Do not have Medicare or Medicaid (except Medicaid Spend-down/Deductible)
• Do not have access to Tribal Health Care
• Do not have access to the Veterans Clinic

You may be eligible for CHAC healthcare services.  If you have questions about your eligibility, please do not hesitate to call one of our offices to speak with an Enrollment Volunteer.  We are happy to answer any questions you may have.
